1. Pursuant to the provisions of reference (a) Subject, hereinafter
referred to as Petitioner, filed enclosure (1) with this Board
requesting, in effect, that the applicable naval record be corrected
to show Petitioner enlisted in the Full Time Support (FTS) program.
2. ‘The Board, consisting of Mr. Zsalman, Mr. George, and
Mr. Ruskin, reviewed Petitioner’s allegations of error and injustice
on 14 April 2014 and, pursuant to its regulations, determined that the
corrective action indicated below should be taken on the available
evidence of record. Documentary material considered by the Board
consisted of the enclosures, naval records, and applicable statutes,
regulations and policies.
3. The Board, having reviewed all the facts of record pertaining to
Petitioner's allegations of error and injustice, finds as follows:
a. Before applying to this Board, Petitioner exhausted all
administrative remedies available under existing law and regulations
within the Department of the Navy.
b. In correspondence attached as enclosure (2), the office
having cognizance over the subject matter addressed in Petitioner’s
application has commented to the effect that the request has merit and
warrants favorable action.
CONCLUSION
Upon review and consideration of all the evidence of record, and
especially in light of the contents of enclosure (2), the Board finds
the existence of an injustice warranting the following corrective
action.

RECOMMENDATION:
That Petitioner's naval record be corrected, where appropriate, to
show that:
a. Record of Military Processing - Armed Forces of the United
States (DD Form 1966/1, MAR 2007), executed on or about 6 May 2011 -
Block A (Service Processing For) is modified to read "DNV" vice "DNR",
b. Enlistment/Reenlistment Document (DD FORM 4/1, OCT 2007),
executed on or about 6 May 2011 - Paragraph 8a (For Enlistment in a
Delayed Entry/Enlistment Program (DEP)) is null and void.
¢. The 4 year Enlistment/Reenlistment Document (DD Form 4/3, OCT
2007), executed on or about 15 June 2011, is nvll and void.
a. The Enlistment Guarantees (Annex "A" to DD Form 4, NAVCRUIT
1133/52), executed on or about 6 May 2011 is modified to read
"Reserve" vice "Active, and Option 1 "HOSPITAL CORPSMAN {HM5YO) CLASS
"A" SCHOOL GUARANTEE - FULL TIME SUPPORT (FTS)" vice "HOSPITAL
CORPSMAN (HM5YO) CLASS "A" SCHOOL GUARANTEE". Note: all other remarks
remain the same. :
e. The 12 month Agreement to Extend Enlistment (NAVPERS
1070/621), executed on or about 15 June 2011, is null and void.
f. The Petitioner executed a 12 month agreement to remain on
active duty (NAVPERS 1070/622), operative on or about 15 June 2015,

TAR "V", TAR-TEMAC "Y¥", EFAOS "14 June 2015", SEAOS "14 June 2016",
RADOMO "048", and EREN "5 May 2019".
g. A copy of this Report of Proceedings will be filed in
Petitioner’s naval record.
4. It is certified that quorum was present at the Board’s review and
deliberations, and that the foregoing is a true and complete record of
the Board's proceedings in the above entitled matter.
